WHAT IS ZERO SUICIDE
About Zero Suicide
The Zero Suicide framework is a system-wide, organizational commitment to safer suicide care in health and behavioral health care systems. The framework is based on the realization that suicidal individuals often fall through the cracks in a sometimes fragmented and distracted health care system. A systematic approach to quality improvement in these settings is both available and necessary.
HOW IT WORKS
Take a Few Easy Steps
Provide education to identified community; support cultural change, to eliminate stigma and resistance to suicide awareness; and suicide prevention strategies.
Lead telehealth provider workshop; prevention, intervention & post intervention training to integrated healthcare network; to manage continuity of care within the system, and to initiate universality of network systems.
Provide direct access to live screeners through application and telehealth; empower members to engage in self-assessment; and educate network providers on how to identify members at risk, using universal, and evidenced based screening tools.
In collaboration with local offices for suicide prevention; lead identified community in the development of a zero suicide community care plan, which utilizes local resources; and telehealth package when local resources are unavailable.
Members presenting with suicidality are directed to a network “safe site”, and provided real time access to integrated telehealth solutions © via the hub; for risk assessment, and immediate link to provider. based on level of risk; the member is transported either by emergency or non-emergent, urgent, to the provider of choice.
There are three transition points: 1) identify to engage; which the member is linked with a network “safe site”, and provided real time access to telehealth. 2) engage to treatment; which the member is assessed, and plan for stabilization includes follow up appointments, and links to community providers and; 3) discharge to community reintegration; which the member is safely transitioned to natural environment, and linked to adequate supports. Desert Rose provides documentation summaries of all transitions, and exchange with network providers.
Healthcare network reports quality assurance measures, with relationship to response times, interventions used, and outcomes; with intent to improve community support, and create a model which is sustainable, sufficient and efficient in achieving zero suicide.
